This form sends your correction request to [email protected], our dedicated channel for provider data corrections. We acknowledge within 1 business day and substantively respond within 5 business days.

What happens next
- Within 1 business day: we acknowledge receipt and assign a tracking reference.
- Within 5 business days: we substantively respond — either confirming the correction, asking for clarification, or explaining why the displayed data matches our CMS source verbatim.
- Within 7 business days: if the inaccuracy is verifiable against CMS, we display a correction notice on your profile.
- If the underlying CMS record is wrong: we direct you to the appropriate CMS dispute channel and continue to display a correction notice until CMS publishes the corrected record. We re-fetch from CMS and update at our next data refresh.

Special cases (use a different channel)
- Identity theft / fraudulent NPI use: email [email protected]. We can suppress the profile within 48 hours of verified notice while you work with CMS to correct the source record.
- DMCA takedown notice or formal legal correspondence: email [email protected].
- Privacy / GDPR / CCPA data subject request: email [email protected] (45-day SLA).
- Press inquiry about a specific provider: email [email protected].
What we cannot do
- We cannot edit your underlying CMS record. NPPES, Open Payments, Medicare Provider Utilization, and PECOS are federal databases — we re-display their content verbatim. To change the source data, you must file the corresponding CMS dispute.
- We cannot remove a profile simply because you would prefer not to be listed. The data is public federal record published under statutory mandate (Sunshine Act §6002 ACA, FOIA) and reproduced under principles of public-interest journalism. (Identity-theft and verified safety threats are different — see Special cases above.)
- We do not accept payment to remove, alter, or favorably present any profile. This is editorial policy.
